Vacation Request Form: Should you need to change your child's schedule temporarily or request a vacation week please fill out this form and submit it to the office.

 

Before/After Care Schedule Form: Required for students in Early Preschool, Preschool, PreK and Kindergarten needing Before/After Care in addition to their school day schedule.

 

Child Medical Statement: Updated annually, this form must be signed by the pediatrician and filled out in its entirety.

 

Administration of Medication: If your child needs medication administered during his/her day at school please fill out this form and submit, along with the medication, to the office. This is the form used for emergency medication, i.e.: Epi-pens and inhalers, as well. Note: Name on medication MUST match name on the form exactly (i.e.: if using Wal-dryl the form must say Wal-dryl and not Benadryl).
